This information guides us on what to screen for when you become part of the FDL primary care family.

Current Medications

Always know your medications and why you are taking them. Some medications may interact with foods or may require surveillance due to known side effects. ⁃For example: If you are taking Lisinopril your Renal function should be checked at least every 3-6 months.

Surgical History

Whether your visit to the ER turned into a visit to the Operating room or if it was a planned procedure. Your primary care physician needs to know about it. The more we know, the better we can take care of you.

As a board-certified family medicine physician, I have had the honor of working at all three-access points to immediate care: The ER, Urgent Care, Clinic. Although urgent care centers or remote Telehealth appointments are quick and easy, you may be seeing multiple providers who only have one piece of your story at that moment in time. Having a primary care physician that does a thorough history and physical exam allows your provider to put all your medical history together. This allows us to best assess and address your current ailments and screen for future problems that may arise due to genetic and lifestyle choices.
